I only really see this with Holby City on BBC1. It records the first showing of the episode just fine on the Tuesday evening (it's recording right now!) but also records the sign language repeat the following Tuesday in the wee small hours.
I'd always assumed this was just down to dodgy guide data incorrectly flagging the repeat as a new episode.

I get this with Xtra Factor (which will re-record on ITV2+1) and Big Brother (which will re-record on Channel 4 +1) - but strangely not every occurance.
This is on UK DVB-T from the Sutton Coldfield transmitter.

Seeing re-runs being recorded when series recording is set to only record new showings - even the guide data shows rerun in it.

Seeing this with "The Daily Show" on Comedy Central, because the guide data is very poor. The guide doesn't indicate the reruns. Solution is to tell media center to only record the episode around a specific time. In my case the new episode of the day airs at 11pm. This has been an issue for me in Vista as well as Windows 7. It's been a while since I set this up, but I believe to get this option you need to schedule the series recording from the guide. I believe if you search and then schedule the series recording, the option to chose the timeslot is not available.

See this occasionally, for example sometimes Family Guy on TOON network will record the same episode multiple times even though the guide data seems to properly list the episodes as being the same. Not sure who is to blame here, MC or something flaky in the guide data.
I also have the problem with the Daily Show that superswiss has and solved it the same way.
The worst one for me is NFL Films Presents on the NFL Network HD (FiOS, Tampa FL) -- some of the shows didn't have episode titles. It has also happened on It's always Sunny in Philadelphia on FX HD.
But I've also seen it on Family Guy where reruns get recorded on channels other than Fox -- I didn't check the guide data to see if they were properly marked as reruns, just ended up telling MC to only record from Fox.
Another possibly related issue is that my keyword recordings don't always work. If I delete them and readd them they will work again, for a while. I never had this problem in Vista and used the exact same keyword recordings.
But none of this is as annoying as the new channels that I don't get that pop up in my guide everday. What is odd is that I use FiOS with an ATI DCT and a few ATSC tuners and the channels that are added that I don't get are DirecTV channels. It is very annoying to get errors saying my shows couldn't record and accidentily changing to channels I don't get.
I only have this problem with a ATSC sub-channel. The shows have all the same generic description and no episode information. They also replay them 2-3 times a day, then the next week there would be a new episode and they replay that over and over.

Pretty much all of Mrs. Lobster's shows on HGTV are recorded all of the time, on every channel (HD and SD) even at the same time. For example...if House Hunters International is on at 8:00 on HGTV-HD and regular HGTV, it records at 8:00 on both channels, even though I have HD preferred as the series settings. I have it set to record reruns as well as first run, but it records the same episode every time it airs, not recognizing that it has already recorded it. Additionally, SyFy's Stargate Universe displays the same behavior as well as The Golf Channel's Big Break Disney Golf and Cartoon Network's Robot Chicken.
The airings on regular network TV seem to be better, but not perfect. The Office only records first run as requested, Fringe does not duplicate recordings....hmmm...those are really the only broacast network shows we record.
