Recording on Wrong HD Channel

Every 2-3 weeks I have an issue where shows I record on HD channels record on another HD channel. Last week Tuesday I recorded the Mentalist on channel 11.1 which is the local CBS channel and it worked fine, but Thursday when I recorded CSI on channel 11.1 it recorded a show from FOX which is channel 17.1. When I went into the guide and clicked on 11.1 it pulled up FOX. When I clicked on the FOX channel it pulled up NBC. I end up having to re-setup the TV channels and re-scan for all QAM HD channels. This works for a few weeks, then it happens again. I usually don't notice it until I go to watch a recorded show and find out it has recorded on the wrong channel. I have all updates applied and also have the TV pack installed.

Is there a permanent fix to this?

I have reformatted my computer and I have put a brand new copy of Windows 7 Home Premium on my computer.  This problem still happens.  I went to watch a recorded CSI and it ended up being a show recorded on Fox.  The channels work for 2 weeks or so, then they end up getting mapped wrong.  Has anybody else had this issue or know of a fix?  Is this an issue with my cable provider?  It only does this on HD QAM channels.
